Birth Certificates

Congratulations on the birth of your new baby!

Did you know?

Every child born in Ontario must be registered with Ontario's Office of the Registrar General. This allows documents such as birth certificates and Social Insurance Numbers to be produced.

Service Ontario's online Newborn Registration Service is the easiest way to register your baby's birth and the fastest way to apply for your baby's birth certificate, social insurance number (SIN) and sign up for Canada child benefits. This 4-in-1 registration process is called the Newborn Bundle.

The hospital will still issue a Birth Registration Form (also known as the Statement of Live Birth). This is your child's permanent record of birth. Registering your child's birth will ensure he or she has a legal identity so that documents such as a birth certificate, Ontario Health Card and Social Insurance Card can be produced without hassle.
